Most of us walk into our annual check-up hoping to hear one word: fine.

But “fine” is a slippery word. Fine doesn’t always mean thriving. Fine doesn’t mean strong, joyful, or ageless. Fine often just means: no obvious fires to put out today.

Longevity isn’t built on “fine.” It’s built on curiosity. On asking better questions. On choosing to see your doctor as a partner in your vitality, not just a gatekeeper of prescriptions.

Here are five questions that can transform a routine appointment into a conversation about living stronger, softer, and longer.

1. “Based on my age and lifestyle, what are my top three risks? And can we run deeper tests to clarify them?”

Globally, the biggest threats are heart disease, cancer, and diabetes (World Health Organization). But your risks depend on you—your family history, your habits, your hormones, even your zip code.

For example, if your doctor sees heart disease as a top concern, you could ask about a Cleerly scan. Unlike a standard calcium score, it doesn’t just look for hardened plaque. It also finds soft plaque—the hidden buildup that can trigger heart attacks (Journal of Cardiovascular Computed Tomography, 2021). Catching it early means you can step in with lifestyle changes before symptoms show up.

2. “Can we go deeper than basic bloodwork—insulin, ApoB, and cortisol along with HbA1c and cholesterol?”

Standard labs skim the surface. To see what’s really happening, go deeper:

ApoB: A stronger predictor of heart disease than LDL cholesterol (JAMA Cardiology, 2019).

Insulin: Early warning for metabolic dysfunction—even before diabetes develops (NIH).

3. “Can we check inflammation markers like C-reactive protein—and even heavy metals?”

Inflammation is the silent accelerant of aging. Scientists call it inflammaging—that constant, low-grade fire that chips away at health over time (Nature Reviews Immunology, 2019).

A simple CRP blood test can reveal if your body is quietly inflamed. Elevated CRP is linked to higher risks of heart disease, diabetes, and Alzheimer’s (Harvard Health).
